To adhere to the written care plan to:

1.        Assist the client with activities of daily living such as bathing, grooming, and nutritional needs.

2.        Assist client with ambulation, transfer and/ or range of motion exercises.

3.        Perform delegated tasks as instructed by the Care Manager or direct supervisor.
